Our purpose

To learn and share together, advocate, measure and communicate about the value of pharmacists in primary care team-based settings.

To contribute to ensuring Ontarians receive safe and cost-effective evidence-based medication therapy using a collaborative team-based approach in primary care.

How Primary Care Team Pharmacists Help

Provide Direct Patient Care
Including general comprehensive medication assessment and review and leading specific programs focused on high-alert medication (e.g. opioids), chronic disease management (e.g. diabetes, hypertension, smoking cessation), and vulnerable patient populations at risk of adverse events (e.g. post-hospitalization, elderly with polypharmacy)

Provide Education
Educate patients, caregivers, family physicians and other health professional learners

Lead or Work as Part of Quality Improvement Teams
Develop organizational process for Medication Reconciliation, participate in Quality Improvement initiatives such as evaluating impact of patient care services, partner with local care partners to integrate care regionally.

Provide Health Care System Navigation
Helping foster linkages in the healthcare system to promote effective and efficient resources utilization, with attention to optimize medication use

Develop Partnerships & Collaboration
Collaborating and creating partnerships with external groups and stakeholders

Lead or Contribute to Development of Local Health Care Policy
Work within the primary care organization or with other organizations to build better approaches to medication management and health care delivery
